Price is a yearly rate from the date you sign up. It is not automatically deducted once the plane expires.

*Family members cannot be substituted in for other family members. Your effective date is the date you sign up. Cleanings must be performed before the end coverage date.
This dental discount plan is NOT dental insurance. It cannot be combined with any other dental insurance.
C-payment is due at the time of service for any work received. Rates are subject to change annually.
Membership dues are non-refundable. Membership is only good at Mebane Oaks Family Dentistry, therefore if you are referred a special discount offered to members will not apply.
